    How to Create a Safety Checklist for Field Workers

    What to include, how to word each line, and how to build a checklist your crews complete honestly at 5am — not one that gets ticked in the ute on the way home.

    What a field safety checklist should cover

    Adapt these sections to the task rather than shipping all of them on every form. A workshop pre-start needs a fraction of what a confined space entry needs.

    Where the obligations come from

    A checklist is a control, not the paperwork

    Under the model WHS Act you must identify hazards and eliminate or minimise risks so far as is reasonably practicable. A field checklist is one of the simplest ways to prove that hazard identification happened at the workface, on the day, by the people exposed to the risk.

    It has to match your SWMS and risk assessment

    Your checklist should draw its lines from the SWMS or JHA for the task. If the SWMS lists five controls for working at height and the checklist asks about two, the gap is visible to any regulator or client auditor reading both documents side by side.

    Mine sites add their own required checks

    Queensland coal and metalliferous operations, WA mines under the WHS (Mines) Regulations and NSW operations under the Resources Regulator all set their own pre-start, take-5, isolation and permit expectations. Build the operator's requirements into the site version of your checklist rather than running two systems.

    The record has to survive the shift

    A completed checklist is evidence of what was assessed and by whom. If it lives on a clipboard in a ute, it will not be there when an incident investigation or client audit asks for it eighteen months later.

    Confirm the detail with Safe Work Australia and your state regulator — Resources Safety & Health Queensland, DEMIRS (WA) or the NSW Resources Regulator — plus the site operator's own safety management system, which is usually stricter than the legal minimum.

    Building the checklist, step by step

    Start from the task, not a template

    List the actual steps of the job — mobilise, isolate, remove guard, lift component, reinstate, test. Hazards attach to steps, so the steps give you the checklist structure.

    Pull the hazards from your own history

    Mine your incident reports, hazard cards and near-miss records from the last twelve months. The hazards your crews actually strike belong at the top of the list, not the generic ones.

    Write each line as a yes/no verification

    'Exclusion zone barricaded and signed' beats 'Consider access control'. A field worker in gloves at 5am needs something they can answer in two seconds and be held to.

    Keep it to one screen or one page

    Aim for 12 to 20 items. Long checklists get pencil-whipped. Anything that is not a live decision on the day belongs in the SWMS, not the daily check.

    Force an action on every 'no'

    A 'no' answer should require a comment, a photo and either a control or a stop-work escalation to the supervisor. A checklist that lets a 'no' pass silently teaches crews it does not matter.

    Trial it with a crew before you roll it out

    Run the draft on two or three real jobs with the people who will use it. They will find the ambiguous wording, the missing hazard and the line that does not apply to their plant.

    Digitise it and attach it to the job

    Complete it on a phone or tablet in the field, offline if there is no signal, with photos, GPS, timestamp and worker sign-on saved straight against the job record.

    Review after every incident and every 12 months

    Add the control that would have prevented the last incident and delete lines nobody has ever ticked 'no' on. A checklist that never changes is a checklist nobody reads.

    Best practice for contractors

    One checklist per task type

    A workshop pre-start, a working-at-height check and a confined space entry are different risks. Build a short, specific version of each rather than one long universal form.

    Put the highest-consequence checks first

    Isolation, exclusion zones and fall protection sit at the top. If a crew only gets through the first half under pressure, make sure that half is the part that kills people.

    Use plain English and site language

    Write it the way the crew talks. If they say 'tag out', don't write 'implement de-energisation controls'.

    Require a photo on the critical items

    Barricading, isolation points and tags photographed at the workface turn a tick into evidence, and lift completion quality overnight.

    Make it work offline

    Most field and mine work happens outside coverage. The form must complete on the device and sync when the worker gets signal.

    Report on what the checklists tell you

    Track how often each line is answered 'no'. That is your live hazard trend, and it is free once the checklists are digital.

    Common mistakes

    Copying a generic template off the internet

    A checklist that does not mention your plant, your site or your hazards signals to crews and auditors alike that nobody thought about the job.

    Making it too long

    Forty items guarantees ticking without reading. Shorter and honest beats comprehensive and fictional.

    Only the supervisor signs

    The workers exposed to the risk should sign on. That is the difference between a briefing and a form filled in at the office.

    No path for a 'no'

    If there is no stop-work or escalation attached to a failed check, the checklist becomes a formality rather than a control.

    Paper only

    Paper checklists get lost, water-damaged, back-dated and never analysed. You lose both the evidence and the trend data.

    Never reviewing it

    Hazards change with the season, the site and the plant. An unrevised checklist slowly stops describing the job being done.
